Bug Description:
For both birth and death events, informant's relationship to child / deceased shows blank on certificate.

Steps to reproduce:

  1. Login as RA/local reg/national reg.
  2. Navigate to ready to print workqueue.
  3. Download any birth/death record.
  4. Click on 'Print'.
  5. Select any template for certificate printing.
  6. Print the certificate.
  7. Observe the informant section in both preview page and downloaded PDF.

Actural Result:
Shows blank for informant's relationship to both child and deceased.

Expected Result:
Should reflect the relationship that was input while creating the declaration.
