- Birth registration form change: The address structure for 'Parish/Ward' and 'Village' fields should not be freetext but a searchable dropdown (typeahead) that is populated with the provided reference data

- Death registration form change: The address structure for 'Parish/Ward' and 'Village' fields should not be freetext but a searchable dropdown (typeahead) that is populated with the provided reference data

- Birth registration form change: Health Facility Administrator should NOT be able to declare a birth event that occurred outside their Health Facility

- Death registration form change: Health Facility Administrator should NOT be able to declare a death event that occurred outsider their Health Facility

- Birth registration form change: Registration Officers should NOT be able to declare a birth event that occurred outside their district

- Death registration form change: Registration Officers should NOT be able to declare a birth event that occurred outside their district

- Birth registration form change: The Notifier should be asked their preferred form of communication and the appropriate field should be shown based on their selection. For example, if they choose 'email' then a field asking for the Email address is shown. If SMS, then a field asking for their Phone Number is shown.

- Death registration form change: The Notifier should be asked their preferred form of communication and the appropriate field should be shown based on their selection.
